Survey Supply, Inc. makes it easy to generate quotes and complete your purchase once your internal approval process is complete.
Step 1: Generate a Quote
Our website allows you to create a quote directly from your shopping cart.
- Add all desired items to your cart.
- Click “Create Quote from Cart” to generate a downloadable quote.
- Use this quote internally to obtain PO approval within your company.
Step 2: Place Your Order
Once your PO has been approved, you can complete your order in one of two ways:
Option 1: Complete checkout on our website
- If you created an account and were logged in when generating your quote, your cart will be saved — no need to re-add your items.
- Simply return to your cart, review your items, and enter your PO Number in the “Purchase Order #” field located in the Shipping Section during checkout.
- Submit your order — no need to email us separately.
Option 2: Email your authorized Purchase Order
- Email your approved PO directly to kevin@surveysupplyinc.com.
- Please include your company name, shipping address, contact information, and any quote reference number (if applicable).
- You may attach the PDF quote you received from our system to ensure accurate order matching and faster processing.
Step 3: Order Processing
Once your PO is received (either through checkout or email), our team will confirm product availability, prepare your order, and send an order acknowledgment. If any adjustments are needed, we’ll contact you promptly before processing.

- Accepted PO Customers: We work with corporate, government, and educational institutions that have established purchasing procedures.
- Payment Terms: Standard NET30 terms are available for approved accounts.
- Need a W-9 or Vendor Setup Form? Email kevin@surveysupplyinc.com and we’ll provide any documentation required to add Survey Supply, Inc. as a vendor.
